H1N1 Vaccine Clinic

8:30-11:00 am

 

Nurses from Pettis County Health Center will be here on Wednesday, December 16th from 8:30 to 11:00 am to administer H1N1 (or swine flu) vaccines.  This vaccine is free of charge for anyone who would like to get one.  This is available to all students at Smithton R-VI, their parents, siblings, and any community member.  Parents do not need to be present for their child to receive this immunization.  A permission form and health screening sheet must be filled out and returned to the school nurse by Friday, December 11th, 2009 for the vaccine to be administered to your child.  If no permission form or health screening sheet are turned in by the clinic date, then your child WILL NOT get vaccinated.  If any parent, sibling, or grandparent would like to come and receive a vaccine please make a small note on your child’s health screening form so that we can keep track of the number of vaccines that will be needed.  If a parent would prefer to be present when their child receives the immunization that will need to be noted on the sheet as well, otherwise your child will receive the vaccine with the rest of the students in their class receiving the vaccine.  If you have any questions regarding the H1N1 vaccine or the procedure for that day please contact the PCHC at 660-827-1130.
